87. Ms Libby Mettam to the Minister for Health: I ask this question on behalf of Stacey Hildebrandt, whose beautiful 18-year-old daughter, Hailee, took her own life as an inpatient of Fiona Stanley Hospital. I refer to comments from the South Metropolitan Health Service, which has already concluded that Hailee's death was unexpected. How could the minister's government already conclude that Hailee's death was unexpected, given that, firstly, her leave from hospital without contact being made with her carer was in clear breach of the recommendations of the 2012 Stokes review of mental health; secondly, she had attempted to take her own life just days earlier; and, thirdly, there had been a code black called involving Hailee at the hospital on the same day she took her life?
Ms Meredith Hammat replied: I thank the member for the question. I want to start by expressing my sincere condolences to Hailee's mother, Stacey, and to all of her family at this time. I cannot imagine how difficult it must be to lose a child in any circumstance, but particularly in those circumstances. Member, I do not intend to use question time to discuss people's personal and private medical history, but I will say this: I am advised that the South Metropolitan Health Service has a senior executive in regular contact with the family. A detailed review is an important part of understanding the facts of the situation and to properly understand what occurred. My understanding is that that review and the findings from it will be shared with the family, as is appropriate. I understand how important that will be to them so that they can understand what occurred in this event, and also for our health services to understand what happened and to identify if there are any learnings that they need to take away from this tragic event. As I say, the findings of that review will be shared with the family. As I have said to many people who have had findings and then wanted to discuss them with me, if they wish to talk to me about them, I am available to do that.
88. Ms Libby Mettam to the Minister for Health: I have a supplementary question. What commitment will the minister make to ensure that the 2012 mental health review recommendations, as well as those from this review, are implemented as a priority?
Ms Meredith Hammat replied: In relation to the Stokes review, the advice that I have is that those specific recommendations have been implemented. Notwithstanding that, clinical practice means that, right across our health system, people are reviewing incidents all the time as they occur and modifying as necessary to ensure the safety and quality of the services that we offer.
